What is TMS therapy?
Transcranial Magnetic Stimulation (TMS) is a non-invasive treatment that uses a magnetic coil placed on the scalp to deliver short magnetic pulses. These pulses create tiny electrical currents in targeted brain circuits, gently nudging them toward healthier activity patterns. It’s performed while you're awake in our office, with no surgery or anesthesia.
TMS is FDA-cleared for conditions including: treatment-resistant major depression, obsessive-compulsive disorder (OCD), certain migraines, and smoking dependence. It may also help with PTSD, anxiety, bipolar depression, chronic pain, and post-stroke rehab, though evidence and coverage for these vary. Most people tolerate it well, with common side effects like scalp discomfort or headache. Serious effects like seizures are rare, and hearing protection is always used.
Your TMS journey: what to expect
Choosing TMS is a significant step, and we make the process clear and supportive from day one. Here’s a typical overview of your experience at California Holistic Health:
- Step 1: Quick intake (phone or online) to assess your needs, prior treatments, and basic safety.
- Step 2: Initial consultation (TMS evaluation) to confirm candidacy, review your history, goals, and discuss what to expect.
- Step 3: Baseline check + personalization (PrTMS® as appropriate) to establish measurable symptom scores and tailor your plan.
- Step 4: First treatment visit, including mapping and setting your personalized stimulation intensity.
- Step 5: Treatment sessions, typically 5 days a week for 4-6 weeks, lasting about 20 minutes each.
- Step 6: Weekly check-ins to monitor progress and make any necessary adjustments.
- Step 7: Completion review to discuss your outcomes, follow-up plans, and relapse prevention strategies.
Common questions about TMS therapy
Will it work for my problem?
TMS is best supported for major depression, especially when standard treatments haven't helped. We also offer it for FDA-cleared indications like OCD, migraine, and smoking dependence. We’ll provide honest information about what’s proven versus emerging for other uses like PTSD or anxiety.
Am I a good candidate?
We carefully screen for suitability, considering factors like prior treatments, history of bipolar disorder/mania, seizures, certain metal implants, and pregnancy. Your consultation will address all concerns to ensure TMS is safe and appropriate for you.
Is it safe? What are the side effects?
Most people tolerate TMS well. Common side effects include temporary headache or scalp discomfort. Seizure risk is very rare (about 1 in 10000 treatments) and mitigated by careful screening. Ear protection is always used to ensure hearing safety.
What does it feel like?
You’ll experience a tapping sensation on your scalp, and possibly some facial twitching during pulses. You can talk, listen to music, or relax during sessions. It’s not claustrophobic as there's no enclosed scanner.
How long does it take, and how many visits?
A typical course involves weekday sessions for several weeks (commonly 4-6). Session length varies but is usually around 20 minutes. Some patients notice changes within 1-2 weeks, others later.
What if it works — how do I keep results?
We create a comprehensive follow-up plan, including coordination with your therapy/medications and lifestyle supports. We’ll discuss maintenance or "booster" sessions if symptoms return, ensuring long-term well-being.
Will my insurance cover it? What will I pay?
Many insurance providers cover TMS for depression. We will assist you with understanding authorizations, copays, deductibles, and your total estimated out-of-pocket costs based on your plan.
Will it change my personality or make me 'not myself'?
No. TMS targets symptoms like mood, energy, and anhedonia, not your core personality. We monitor closely for unwanted changes, ensuring the treatment enhances your well-being without altering who you are.
Do I have to stop medications or therapy?
In many cases, TMS is done alongside ongoing medication and therapy. We will coordinate with your current providers to create an individualized plan that integrates seamlessly with your existing care.
Why choose your clinic instead of another?
We pride ourselves on personalized mapping/dosing, measurement-based progress tracking, an experienced team, rigorous safety screening, and clear communication in a supportive environment. We prioritize your comfort and results.
